Butterick 4728 sewing pattern, dated 2006, now out of print.

Misses' / Misses' Petite Jacket and Dress:

Above waist, unlined jacket has collar and collar band, three-quarter sleeves and stitched hems. Lined dress, mid-knee has fitted boned bodice and princess seams, below waist skirt with pleats, shoulder straps and back zipper closure.

Sizes   6 - 8 - 10 - 12:

Bust  30.5 - 31.5 - 32.5 - 34 inches / 78 - 80 - 83 - 87 cm

Pattern is uncut and factory folded. Envelope is in very good condition.
