Simplicity 7494 Vintage Sewing Pattern, dated 1986. 

Misses' Easy-To-Sew Dresses in Three Lengths:  

V. 1, 2 and 3 have front and back scoop neckline. V. 4 and 5 have shoulder straps and back elastic casing. V. 3 and 5 have skirt gathered to bodice at dropped waistline seam and attached self belt. 

Sizes  16 - 18 - 20:

Bust   38 - 40 - 42 inches / 97 - 102 - 107 cm

Waist 30 - 32 - 34 inches / 76 - 81 - 87 cm

Hip    40 - 42 - 44 inches / 102 - 107 - 112 cm

Pattern is uncut and factory folded. Envelope has some shelf wear.
