Simplicity 5721 Vintage Sewing Pattern, dated 1982. 

Misses' Asymmetric Dress with Quilted or Plain Yoke:

Dress gathered to asymmetrical yoke has left shoulder button closing, side seam pockets and set-in sleeves gathered to armholes. V. 1 has self quilted yoke, corded piping trim and elbow length sleeves with elastic casings at lower edge. Ribbon trimmed V. 2 has stand-up collar, lower ruffle and long sleeves gathered to buttoned cuffs. V. 3 has contrasting pre-quilted yoke, and cuffs, piping trim and long sleeves.

Hip:     40 inches / 102 cm

Pattern is uncut and factory folded. Envelope is in very good vintage condition.
