Butterick 5274 vintage sewing pattern, dated circa 1950. 

On or Off-The-Shoulder Blouse: Eyelet Trim.

"Quick & Easy" blouse for sun time or dancing can be worn on or off the shoulders. Eyelet outlines the neckline which has a drawstring adjustment. (A) Eyelet trim on puffed sleeves. (C) Layers of eyelet on sleeves form a flounce.

Waist 28 inches / 71 cm

Hip    37 inches / 94 cm

Pattern is UNPRINTED tissue and appears unused. I have gently unfolded the pieces to confirm completeness. Envelope is in good vintage condition, with age appropriate paper tanning, and a store stamp on the front.
